چکیده

Noncommunicable diseases (NCD)––cardiovascular diseases (CVD), diabetes mellitus, cancer, and chronic obstructive pulmonary diseases––account for two-thirds of death and disability, globally, every year [1]. CVD alone, including coronary artery and cerebrovascular disease, accounts for 30% of the global burden of disease. As many lowand middle-income countries (LMIC) progress through the epidemiologic transition and life expectancy increases, they are also undergoing globalization as well as economic reform [2,3]. This globalization, which is coupled with urbanization and modernization, contributes to changes in the lifestyle and behavior of people living in LMIC and emerging economies (such as the former Soviet Union and Eastern European countries). These factors play a role in the heavy burden of NCD, as much as 80%, that now occurs in these countries [3]. Over the last three decades, a long list of heart health declarations and policy documents (Table 1) have articulated and emphasized public policy as a means of CVD prevention and control [4–37]. The policies and programs described in these documents, many of which deal with health education and environmental change, have played important roles in the prevention and control of heart disease and stroke in high-income countries, providing resources and authority for essential public health functions to be mobilized. There is wide consensus that the global community has to help build more resilient health systems. But do we really know what resilience means, and do we all have the same vision of resilience? The present paper presents a new conceptual framework on go...

Policy Capacity in the Learning Healthcare System; Comment on “Health Reform Requires Policy Capacity”

Pierre-Gerlier Forest and his colleagues make a strong argument for the need to expand policy capacity among healthcare actors. In this commentary, I develop an additional argument in support of Forest et al view. Forest et al rightly point to the need to have embedded policy experts to successfully translate healthcare reform policy into healthcare change. Translation of externally generated i...

Agency, Structure and the Power of Global Health Networks

Global health networks—webs of individuals and organizations linked by a shared concern for a particular condition—have proliferated over the past quarter century. In a recent editorial in this journal, I presented evidence that their effectiveness in addressing four challenges—problem definition, positioning, coalitionbuilding and governance—shapes their ability to influence policy. The editor...

Policy Capacity for Health Reform: Necessary but Insufficient; Comment on “Health Reform Requires Policy Capacity”

Forest and colleagues have persuasively made the case that policy capacity is a fundamental prerequisite to health reform. They offer a comprehensive life-cycle definition of policy capacity and stress that it involves much more than problem identification and option development. I would like to offer a Canadian perspective. If we define health reform as re-orienting the health system from acut...
